- Pick 3 Different Security Questions and Answers
- Pick a Site ID Image and Phrase
- Create a Username and Password
- Enter the Activation Code provided by Orthopedic Institute of Pennsylvania
Note: Activation Codes are case sensitive and contain uppercase and lowercase letters, mixed with numbers. If you are unsure of the case of your Activation Code please call us to confirm. If you experience issues activating your account please call us at 836-1512.
On the Patient Portal login page, enter your Username and click Login.
Confirm your Site ID Image and Phrase are correct, enter your Password and click Login

Follow Route 3 northbound to exit 38. Travel south on Route 18 south . At the third traffic light, turn left on to Columbian Street. Take the first left. Parking garage is at the end of the driveway on the right.

Shuttle
The Courtesy Shuttle provides transportation to patients who need a ride to an appointment at South Shore Hospital or to a program or service directly affiliated with South Shore Hospital. The Courtesy Shuttle does not deliver patients to physician offices. For more information, call 781-624-4350.

Please note, if you are requesting a login reset for your patient portal, activation codes are mailed to you through USPS. Virtual Care At South Shore Medical Center
Patients at South Shore Medical Center have access to virtual visits with their primary care provider and with providers in a number of specialties.
Virtual visits are conducted via Zoom using a smartphone, tablet, or laptop.
These visits are scheduled in advance, similar to an in-person visit, but allow patients to see familiar providers from the comfort of home.
- Current patients: Schedule an appointment in South Shore MyChart or call 781-878-5200.
- New patients: Several South Shore Medical Center providers are accepting new patients. To become a patient, fill out our New Patient Registration Form and a member of our team will contact you to get you set up.
Need Help With The Patient Portal
