If your pain is affecting your ability to perform your daily activities, your doctor may recommend a nonsurgical treatment such as:

  • Staying off the affected foot and pausing strenuous exercise is imperative during rehabilitation.

Common Causes Of Peroneal Tendonitis In Runners

Since peroneal tendonitis is so rare, there arent any large-scale studies on risk factors. The only reports extant in the medical literature analyze a relatively small number of cases.

However, one risk factor does seem to stand out.

A review of twenty-two cases of peroneal tendonitis by Clayton B. Brandes and Ronald W. Smith revealed that over eighty percent of these patients had a high-arched foot. According to the authors, feet with higher arches put more tension on the peroneal tendons, predisposing them to injury.2

Research published in 1993 also suggests that faster running speeds could put extra stress on the peroneal tendons.

Lyle Reber, Jacquelin Perry, and Marilyn Pink at the Centinela Hospital Biomechanics Laboratory in California investigated how muscular activation patterns in the muscles surrounding the ankle changed as a function of running speed in a group of healthy, injury-free distance runners.4

The researchers found that higher running speeds created a significant increase in activity in the peroneus brevis muscle. The intensity of the muscle contraction doubled when comparing an easy jog to race-pace running.

Peroneal Tendonitis Or Peroneal Tendon Tear

There are 3 peroneal tendons that run along the outer side of the shin bone, across the lateral ankle, and connecting to the outer and underside of the foot. Their main function is to turn the foot out and down while acting as a stabiliser of the foot and ankle through motion. If they become overused through repetitive motion such as walking or running they can become inflamed and painful leading to Peroneal Tendonitis. Similarly, a sudden inversion of an ankle can cause a Peroneal Tendon Tear.

In the acute phase, Peroneal Tendonitis may be painful with all movements. In less acute episodes, it may be stiff and tender in the morning. During the activity, it may be tender initially while easing as it warms up. As the condition becomes more chronic it can become constantly painful. Peroneal Tendonitis is a common cause of outside ankle pain without swelling.

Treatment with a Physical Therapist or a Sports Medicine professional is recommended. Bursitis In The Foot Or Ankle

Bursitis causes pain and inflammation, commonly around hard-working joints such as in the big toe, ball of the foot, heel, or ankle. It can also affect any part of the body.

It is caused by swelling in a small pocket of fluid, known as a bursa, or bursae if more than one is affected. Normally these small pouches cushion and protect your bones, joints, and tendons from impact, rubbing or pressure.

However, repeated rubbing, friction, excessive pressure, or injury can cause the fluid inside to increase making the affected bursa inflamed and tender.

For example, you can get bursitis between your ankle and heel if you keep wearing shoes that rub, pinch, or press down on the area. Shoes that are too tight and high heels regularly cause bursitis.

It often appears as a red, painful, swollen lump. However, it can also cause general swelling that you only notice because of the pain, for instance on the bottom of your heel.
